Students' medical information - forms and updating information
Parents/carers of all students are asked to complete a data sheet and ESF visits consent form when they enrol. The forms include a section to record medical information.
Important: As parents/carers you are required to keep the Academy informed of any changes to medical information.

NHS School Nursing Service
The Academy's facilities are used to host several services provided by NHS School Nursing.

Vaccinations
The Academy liaises with School Nursing to host and facilitate immunisations.
Important: Please note that any communication from parents/carers/students regarding vaccinations must be through direct contact with School Nursing and not through the Academy; this includes information regarding consent.

Consent Prior to the vaccinations you will be sent a link to a letter which will include details about consent. You can allow, refuse or retract consent - please do not fail to complete consent as it creates additional and unnecessary work for School Nursing. As mentioned above, any queries, including about consent must be made directly with School Nursing. |

Health and Well-being confidential visits in school by School Nursing
School Nurses are in school for regular confidential appointments. Students can see posters in school for details or speak to the Learning Mentors.
